School of Education - Student Teaching Application Process
 
The Student Teaching Application will be submitted through your Foliotek account in addition to submitting a hard copy of each component.

After uploading all materials of your application to Foliotek, you must present a hard copy of each application component to Jackie Turner, by the deadline:
August 31st for Spring semester
January 31st for Fall semester

Foliotek Registration information and codes can be found in the Education Office, Roemer 106, or by contacting the Foliotek Support Advisor, Dr. Vanessa Coe, vcoe@lindenwood.edu.

Student Teaching Application Components:

  1. Student Teacher Candidate’s Information Form.

  2. Student Teaching Application.

  3. Philosophy of Education

    Write a personal statement reflecting your views on teaching and learning, your influences and inspirations, your strengths and attributes, and your plans to further your education. Keep this statement 1-2 pages in length.

  4. Signed Advising/Program Planning Sheet

    Schedule an appointment with your academic advisor for verification of eligibility to student teach. Your Education advisor will be checking for completion of coursework prior to student teaching as well as ensuring that you have successfully passed all Education courses with a grade no lower than a ‘C’.

  5. Application for Degree Signed by Advisor– Xeroxed Copy

    This document is required if you plan to graduate upon the conclusion of Student Teaching. This item is optional for MAT students. If you do no plan on graduating after student teaching, please include a note stating that a degree is not being pursued after student teaching at this time.

  6. Passing CBase Score Report – Undergraduates Only

  7. Passing Praxis II Score Report

  8. Negative TB Test Results

    The TB test is valid for 6 months. Students may turn in the TB test when the application is due and the Placement office will send the date from the TB test to the school district of their choice. It will be at the discretion of the school district to determine whether or not they want the student teacher to get another TB test conducted prior to student teaching.

    Students who pay a health fee through tuition may be eligible to receive free TB testing at Lindenwood’s Health Center. Call to determine eligibility: 636-949-4804.

    Student Teachers may first submit a letter of intent verifying plans to get the TB test toward the middle to end of February for fall placements and the middle to end of September for spring placements. Requests will not be presented to school districts until those results are submitted.

    TB tests can be distributed through your doctor’s office or through any urgent care, walk-in clinic, or the St. Charles County Division of Public Health

  9. Signed Letters of Recommendation (2)

    May range from Academic, Employment, and/or Character References. Please include at least one recommendation from someone who has observed you in a school setting or leadership role.

  10. Missouri Highway Patrol Fingerprint

    Students must schedule an appointment to get fingerprints taken through the Missouri State Highway Patrol. First, to to the website www.machs.mo.gov and select the option requiring a 4 digit code. Enter the code 5662, then click "Populate". Enter personal info and click "Register". Students will be provided with a Transaction Control Number (TCN)-- keep this number in your records; it can be used to track your prints if the need arises. Click "Schedule Fingerprinting".

    Students will have the option to pay online ($44.80) or at the time of the appointment. Please call 3M/Cogent directly 1-877-862-2425 or the Missouri Highway Patrol 1-573-526-6312 for inquiries regarding this process.

  11. Family Care Safety Background Clearance – Dated no earlier than 1 year prior to Student
    Teaching. – Student receives results.

    Family Care Safety Registry

    New applicants must pay a $10 processing fee.

    If you are already on file with FCSR, simply call the number indicated on the website to request that an updated report be sent to the home address of the applicant.

  12. Travel Waiver - Sign and date the Travel Waiver, which states that Lindenwood University will not be held responsible for activities that require the student teacher to travel off campus.

  13. Disposition Assessment - Print off the Disposition Assessment Procedures document and complete these forms with your School of Education academic advisor.



To Submit Your Completed Student Teaching Application, students must upload all components to Foliotek then present a hard copy of each item by the indicated deadline.
  • Once hard copies of applications are submitted, the Student Teaching Placement Coordinator will search by student name to review the application through Foliotek.

Please be advised, there will be a Mandatory Student Teaching Informational Meeting the semester prior to student teaching.
  • The Mandatory Informational Meeting is typically scheduled in April for Fall Student Teaching and November for Spring Student Teaching. The date will be announced to Student Teaching applicants via e-mail.

The weekly accompanying Student Teaching Seminar classes are scheduled to begin on the main campus the first week of January for Spring Student Teaching or the second week of August for Fall Student Teaching, which is before Lindenwood’s semester typically begins.
  • Attendance of each Seminar class is essential and mandatory.
